The Powell Police Department is dedicated to providing citizens and visitors with the information that they want and need. Many of the forms and documents that can be obtained at the police department are also available electronically here on the PPD website. We are working on converting many of these documents into a PDF form that can be completed and emailed to the police department.

Below is a list of forms and documents that can you download and print at any time.

Voluntary Statement Form

If you are asked to provide a statement for a case you were involved in or if you were not asked but would like to make a statement about a case or incident, you can use this fillable PDF form.

Records Request Form

In order to obtain a police report, a request form will have to be completed and submitted. It will then be determined if the case has been adjudicated. If the case has not been adjudicated, the report will not be released.
